All applicants for endorsement can either fill out the online form below or submit in a completed CE Course Endorsement Form document and submitting it to us by mailing it to our address listed on our Contact Us webpage. Applications which are incomplete will be returned to the provider with an explanation and recommendation for submitting needed information. Incomplete applications will significantly delay the review and approval process. If approved, then the approval period is valid for a five year period.
