Policy on Replacement of Certificates & ID Cards

CONTINUING EDUCATION CERTIFICATES

1. All participants who successfully complete a continuing education course receive a course certificate of completion by via email.

2. If you need a replacement CE Certificate of Completion, please e-mail admn@aihcp.org and request one.

3. Include your student ID, your full name as well as the course(s) CE Certificates you would like replacements for.  Your replacement CE Certificate(s) will be e-mailed out as soon as possible.

 

CERTIFICATION CERTIFICATES

1. All who become certified through AIHCP, Inc. and its subdivisions, receive a Certificate of Certification by postal mail.

2. Participants may request a additional certificate(s) or a replacement certificate if needed. The cost of a replacement Certification certificate is $10.00 per certificate plus an additional $10.00 for shipping and handling.

Requests can be made by email to admn@aihcp.org or by mail to:

When you reach the “This Payment is For” section, select “Paying Invoice”. Input $20.00. (Shipping is $10.00 for a Certification.)

If you prefer, you can also pay by check. A personal check or money order is payable to: AIHCP 

3. Once a request is received along with payment, AIHCP, Inc. will prepare and postal mail the requested replacement certificate within 7 business days.

If you wish to replace both the Certification and ID Card, the total will be $35.00. 

WALLET ID CARDS

1. All members who become certified receive a Wallet ID Card attesting to their certification by the AIHCP, Inc., or one of its Subdivisions. This card is provided in the certification package that members receive.

2. Members may purchase an additional Wallet ID Card as needed for replacement. The cost of a replacement ID Card is $ 15.00.
